Overview
Setmelanotide is a melanocortin-4 receptor agonist prescribed under the Imcivree brand for specific forms of obesity. Its distinctive feature is patient selection. It is not an ordinary weight-loss option simply because it influences appetite. The diagnosis underlying the weight problem is central to whether its clinical evidence applies.
Community Use
No exact setmelanotide protocol or profile was verified on the two requested community sites for this article. Broad peptide weight-loss lists are not treated as evidence for routine use. Discussion of this medicine should begin with the current Imcivree prescribing information, including its diagnosis-specific limitations.

| Route / formulation | Dose / reported phases | Frequency | Duration / break | Notes and source |
|---|---|---|---|---|
| Subcutaneous injection | Initial: BBS or POMC/PCSK1/LEPR deficiency, age 12 or older: 2 mg daily for 2 weeks. Acquired hypothalamic obesity, adults and age 6 or older: 0.5 mg daily for 2 weeks. Recommended maintenance: 3 mg daily for adults and children age 6 or older; dose reductions and titration depend on tolerability and renal function. | Once daily | Long-term weight reduction and maintenance for the label's specific indications Break: No informal off-cycle established by this source | FDA prescribing information Imcivree FDA prescribing information, sections 2.2–2.4 (DailyMed) |
Important context: The starting regimen depends on indication, age and kidney function. Younger children have separate dosing. A universal obesity starting/average/maximum schedule is inappropriate.

The current label sets different starting regimens according to age and indication, with additional clinical considerations. The separate table is a reference, not a universal starting-dose guide. A generic adult obesity chart cannot determine how setmelanotide should be prescribed to a person with hypothalamic or genetic disease.
Research Findings
The clinical studies in the label concern defined biological diagnoses and measure outcomes such as weight and hunger. Those inclusion criteria are part of the evidence, not an administrative detail. A finding in a selected rare-disease group cannot simply be applied to people whose obesity has a different cause.
U.S. Approval & Access
The March 2026 label includes acquired hypothalamic obesity, Bardet–Biedl syndrome and specified POMC, PCSK1 or LEPR deficiencies, with age-specific criteria. It explicitly excludes general polygenic obesity. A current diagnosis and appropriate testing or clinical assessment matter; a desire for strong appetite control does not establish labeled eligibility. DailyMed’s product record lists DEA Schedule: None. Being unscheduled does not establish lawful access to an unapproved preparation.
What It Does in the Body
MC4 receptor signaling is involved in energy-balance and hunger pathways. Setmelanotide acts at that receptor, which helps explain its development for particular disruptions of the pathway. A mechanistic connection does not diagnose the cause of someone’s weight gain. That distinction is why evaluation can be more important than comparing advertised weight-loss percentages. The label describes an approximately 11-hour effective elimination half-life. That number does not establish when a person will notice hunger changes, reach a peak benefit or retain a response after stopping.
Benefits & Reported Experiences
Benefits should be interpreted within the diagnosed population and measured clinical endpoints. Hunger reduction and weight change may both matter, but an isolated result is not a guarantee. A care team needs a baseline and follow-up plan appropriate to the condition, rather than measuring success against a general wellness transformation story.
Side Effects & Risks
Label concerns include skin pigmentation and changes in nevi, sexual-arousal effects, depression or suicidal ideation and serious hypersensitivity. The current label also describes additional monitoring relevant to acquired hypothalamic obesity. Discuss changes with the treating specialist; a medicine’s targeted mechanism does not make it free from systemic effects.
Delivery Options
Imcivree is a prescription subcutaneous product. A dosing reference does not replace product-specific teaching from the specialist and pharmacist. This guide gives no injection or preparation instructions. Because the relevant patients and age groups differ, a caregiver’s education may be a substantial part of the clinical plan.
Product Quality & Storage
Verify the diagnosis, exact prescription and follow-up responsibility before discussing access. A routine obesity questionnaire is not a substitute for the label’s selection criteria. Quality also includes the continuity of specialist care: who reviews mood, skin findings and other condition-specific concerns matters alongside the supplied medicine. IMCIVREE’s label specifies refrigerated unopened vials at 2–8°C in the original carton. An opened vial is discarded after 30 days; its exact label defines permitted temperature excursions.
Combinations & Interactions
The label’s indication does not establish a setmelanotide-plus-GLP-1 or growth-peptide stack. Related appetite mechanisms do not prove additive benefit or safety. Existing medications should be reviewed by the treating specialist, and this guide supplies no experimental combination schedule or technique for extending treatment beyond the studied populations.
Talking With Your Provider
If your history suggests a rare genetic or acquired hypothalamic cause of obesity, ask whether specialist assessment is appropriate. Bring existing diagnoses, relevant test results and treatment records. For general weight-care goals, the clinician may discuss other established options rather than pursuing a medicine whose approved use does not match the diagnosis.

Questions worth bringing to your appointment.
- Are there alternatives that address my goal?
- If treatment is appropriate, what monitoring and follow-up would I need?
- Could my health history or current medicines change the risks?
A clinician may recommend a different option—or no medication—after assessing your individual circumstances.
