Research topic. LL-37 is not an FDA-approved general immune, infection or wound-healing therapy. This page does not offer LL-37 treatment.
What makes it interesting.
LL-37 is a human cathelicidin peptide involved in innate immune biology. Experiments examine antimicrobial activity, inflammatory signaling and tissue-repair processes. An endogenous peptide’s biological role does not establish that adding a manufactured version is beneficial or safe.
The story behind the interest.
LL-37 attracted clinical interest because of its role in barrier tissues and immune responses. Human trials have investigated specific topical wound preparations. This is a more focused research history than the broad online claims about systemic immune enhancement or treating persistent infections.
Evidence before expectations
Read beyond
the headline.
A useful research discussion includes the study design, the population and the outcome measured.
Read the original sourcesA 2021 phase IIb study enrolled 148 patients with difficult-to-heal venous leg ulcers. Topical LL-37 was used alongside compression therapy. The full study population did not show a significant improvement over placebo; a post hoc large-wound subgroup finding required further study.
A separate 2023 randomized trial studied LL-37 cream in diabetic foot ulcers and reported improved healing measures, without the expected reduction in the measured inflammatory markers or bacterial colonization. That result is specific to a topical product and wound population.
These studies do not validate injectable LL-37 for chronic infections, general immune support or wellness. For a wound or infection concern, identifying the cause and arranging appropriate clinical care are more useful than applying broad claims from another research setting.
| Research setting | What the record shows | What it does not establish |
|---|---|---|
| Venous leg-ulcer trial | A 148-patient study found no significant full-cohort healing benefit. | A subgroup observation is not a confirmed general benefit. |
| Diabetic foot-ulcer trial | A topical cream study reported improved healing measures. | Does not validate systemic injection or infection treatment. |
| Immune biology | LL-37 participates in innate immune signaling. | Natural occurrence does not establish treatment safety. |

Research doses describe a specific study, not a treatment plan. Online popularity does not establish safety.

Risks & concerns
What deserves attention.
- FDA cites limited safety data and nonclinical reproductive and tumor-related concerns.
- Immune activity is context dependent; more activity does not automatically mean a better clinical outcome.
- Wounds, fever or infection symptoms need appropriate evaluation. Research compounds should not replace prescribed infection or wound care.
A compound’s name, a purity claim or research-use labeling does not establish patient-ready quality, sterility or a reliable clinical outcome.
Regulatory context
Review is not an approval.
LL-37 appears in FDA’s current withdrawn-nomination safety table. It was not on the July 2026 seven-substance agenda. A nomination change does not constitute an approved treatment.
Compounding review, permission to conduct research and approval of a finished drug are different decisions. This page is an invitation to learn and discuss appropriate care, not a statement that this compound can be supplied.

You can be curious about research and still choose a practical care path available today.
Is LL-37 an antibiotic?
LL-37 has antimicrobial activity in experimental research, but this does not make an unapproved preparation an established antibiotic or a substitute for appropriate infection treatment.
Do the wound studies support an injection?
No. The cited human trials studied topical preparations in particular wound populations. Their findings cannot establish the safety or effectiveness of systemic injection.
Does the venous-ulcer trial prove it works?
The main analysis did not show significant healing improvement across the whole study population. The favorable large-wound subgroup analysis was exploratory and needed a further adequately designed study.
Can Seine Bio help me understand the research?
Yes. Start an inquiry about your goals. The team can explain the appropriate clinical discussion and available care options. Active infection or worsening wounds should be assessed promptly by an appropriate clinician.
